This service is used when clients are discharged home from hospital and need help with safety in the home, accessibility in the home, and coordination of services.
The assessment may involve determining the need for:
- Adaptive equipment
- Safety equipment or services
- Home modification
- Home making, nursing care, and other support service requirements
- Other rehabilitation needs
Appointments are often made before discharge. For complex cases, we attend team meetings in the hospital or rehabilitation center before the individual is sent home. When possible, a home assessment is completed before discharge.
